South Texas Clinical Partners ACO is a physician-led, hospital-sponsored Track 2 ACO that:

  • Participates in the Medicare Shared Savings Program
  • Qualifies for MACRA incentives as an Advanced Alternative Payment Model
  • Is dedicated to improving the patient experience
  • Provides high quality, coordinated care focused on improving the health and well-being of the communities we serve
  • Is coordinated by Imperium Health, a leading ACO enablement and practice transformation company
  • Headed by Dr. Monzer Yazji, the group’s CMO who is nationally recognized for ACO performance

Governance and Financial Structure of the ACO

  • Physician practices control 75% of the ACO governance
  • The ACO Executive, Chief Medical Officer and Committee chair positions are controlled by members of the physician leadership and are representatives of the participating practices
  • 70% of shared savings revenue will be distributed to participating practice and 30% to UHS, the ACO sponsor
  • The hospital sponsor will fund 100% of the ACO’s development and coordination costs to promote quality improvement and successful quality reporting.

Coordination of Care

  • The hospital will provide admission, discharge and transfer feeds to ACO participant practices and will facilitate transitional care coordination follow up visits with the PCP
  • The hospital will assist with the reduction of avoidable emergency room and in-patient utilization, as well as readmissions and ambulatory care sensitive condition admissions
  • The ACO will provide high-opportunity patient lists and gaps in care reports
  • The ACO will provide training and operational support to promote quality improvement and successful quality reporting

Benefits of MACRA Eligibility Via Participation in an Advanced Alternative Payment Model

  • Exclusion from Merit-based Incentive Payment System (MIPS) reporting
  • Qualified professionals who receive 25% or more of their revenue from Medicare payments or have a patient panel of 20% or more fee-for-service
  • Qualifying professionals earn a 5% incentive payment in 2019 beginning 2019 and continuing through 2024
